  • Work-Related Injury & Illness

    Our Risk Management department is here to take all first reports of injury and or illness from our clients’ employees.   

    Our staff will contact the medical facility to authorize treatment and drug screening, and we will stay in touch with your injured employee to facilitate a full recovery and prompt return to work.  

    Furthermore, we will file the requisite state notification for first report of injury. We provide assistance with OSHA record keeping and forms such as the Injury and Illness Incident Report (OSHA Form 301), the Log of Work-Related Injuries & Illnesses (OSHA Form 300) and the Annual Summary of Work-Related Injuries and Illnesses (OSHA Form 300A). 

    Finally, our safety team can assist your company with completing an analysis of the work-related injuries or illnesses. Our safety professionals’ efforts will be directed towards identifying all possible causes and contributing factors; and to provide recommendations to reduce the possibility of future occurrences.

  • Certificate of Insurance

    Our Risk Management Department can provide a Certificate of Insurance (COI) for proof of Workers' Compensation Insurance.   

    This form ensures that our team has the correct  information to issue the COI accurately and in a timely manner, in which to satisfy your client (the certificate / policy holder). Additionally, the form requests how and to whom the COI should be sent, and if it should be renewed the next policy year. All of this information is necessary for both the initial issuance as well as future renewals.
